We killed Onyxia on Wednesday, July 6th, and no head dropped. We got:
1) dragonstalker's helm
2) helmet of the ten storms
3) deathbringer
4) eskhandar's collar
5) mature black dragon sinew
and of course, the onyxia hide back. however, she was headless. we got no head drop. it was ffa loot and everyone checked the corpse.
I petitioned a GM and got the following response:
"Answer: We have thoroughly investigated what you reported and found the following: We see that the Backpack off Onyxia did drop when Onyxia died, which mean's that Onyxia's Head would not have dropped. Unfortunately since the Backpack did drop we will not be able to provide the reimbursement of Onyxia's Head."
Uhhhhhh.. did I miss the memo or something? Since when does getting a bag prevent you from getting a head? It's pretty annoying getting brushed off by GM's like this.

Ok, here's the deal. The head should drop 100% of the time.
If you did not receive the head, please petition the GM staff with the date and approximate time of your kill. It might take them a while reimburse (because they need to do some research) but they will reimburse the head.
Apolgies for this bug and thanks for your patience in the meantime. We'll try to get it fixed ASAP.

I also sent a link to Customer Service. Hopefully they can get you reimbursed soon.
